Discovery finds broken foam, still GO to launch

I have recently been reading this book "Will your Next Step be Fatal". Authored by Robert Mittelstaedt, the Dean of Carey School of Busines, the book talks about the mistakes made by organizations large and small that proved fatal and how one can avoid them. One of the storices revolved around NASA and its better known disasters, Columbia and Challenger. The resent news of finding broken foam on the shuttle reminded me of the 'culture' of NASA for 'launch unless proven broken' that led them to these disasters. The author even suggested replacing the whole upper management of NASA to get rid of this 'culture'.
